Joint Assessment Clinics

There is an emphasis that children who have Special Educational needs or complex health needs should receive co-ordinated, high quality child and family-centred services which are based on assessed needs, promote social inclusion and which, wherever possible, enable families to live ordinary lives.

It is recognised that early identification leads to early intervention and support for families and children. It emphasises that this strengthens the ability of families to provide effective support for their children and improves outcomes for the whole family.

This intervention should be a linked series of events that is responsive to family need which follows up identification with information and practical help.

The multi-agency panel will identify children who are not known by agencies, however their presentation gives cause for concern. To coordinate service delivery and avoid duplication of appointments so that we can support the ‘telling the story once’ a child may be given a joint clinic appointment.

Joint Assessment Clinic Process

  • Child referred to Multi agency panel often by Health visitor/setting
  • Information coordinated though panel as to needs
  • Date given for joint assessment
  • Appointment sent
  • Follow up telephone call
  • Home visit prior to appointment if longer than6 weeks
  • Full assessment by teacher /Consultant Paediatrician
  • Involvement with other services if required
  • Detailed assessment report and interventions
  • Follow up by individual service
