NHS Continuing Healthcare - Adults (Ipswich & East Suffolk & West Suffolk CCGs)
The service is for any individual who is registered with a GP within the NHS Ipswich and East Suffolk or West Suffolk CCG area.
The service assesses individuals over the age of 18 for their eligibility to receive NHS Continuing Healthcare. The criteria applied are the National Framework for NHS Continuing Healthcare, which is revised and updated periodically by the Department of Health.
Individuals clinically judged to be eligible to receive NHS Continuing Healthcare have a package of care commissioned for them by the CCG.
Care commissioned can be in any setting and may be a formally arranged package of care in an individual’s own home or care provided in a residential setting. Alternatively, individuals in receipt of NHS Continuing Healthcare have a right to have a Personal Health Budget.
