The Shared Lives Scheme matches vulnerable adults aged 18 and upwards with Shared LIves carers.

Shared Lives may be able to provide support for older people, people with Dementia, a physical/learning disability, mental health issues, Home from Hospital. This can be for day support, short breaks or long term within the carer's own home.

Shared Lives can provide a chance to learn new things, a chance to make new new friends and/or the opportunity to live as part of a family - they can tailor to your needs! Referrals are made to the scheme via the Care Manager in Social Services or by Health Services through the purchasing authority.
