Heywood and Middleton Community Mental Health Team
We offer mental health services to residents of Heywood and Middleton aged 18 to 65 whose mental health needs call for services under the Care Programme Approach (CPA).
Service users will be offered assessment, care planning, care and treatment with usually a named care co-ordinator and Consultant Psychiatrist involved in their care. The team comprises of doctors, nurses, social workers, occupational therapists, support workers and with input from psychological services.
Referrals are to be sent through Pennine Care NHS Foundation Trust Single Point of Access.
