Heywood, Middleton and Rochdale Merit Team

We provide mental health assessment and monitoring for people aged 65 and over. Younger people with a progressive dementia can also receive input from the team.

The team comprises of eight mental health nurses, a mental health occupational therapist, administrator and team manager. We work closely with the older age consultants based at the Birch Hill and Royal Oldham Hospitals.

All referrals are made via the Single Point of Entry Service who screen and direct referrals to the most appropriate team within the Mental Health Service. Referrals can be made by:

• GPs
• Health professional
• Social care professional
