Recovery and Reablement Service

About

The service aims to work with people who face multiple disadvantage and intersecting inequalities including substance misuse, mental health issues (diagnosed and undiagnosed), CSE /ACE/homelessness and being involved in the criminal justice system.

What we do

The Service will work with adults who lead complex lives who may be difficult to engage/hard to reach and have a history of or experiencing any of the following:

  • Multiple Disadvantage and/or
  • Child sexual exploitation (CSE) and/or
  • Adult criminal exploitation (ACE)
  • A diagnosed or undiagnosed mental health issue and/or a substance/alcohol problem.
Our service aims
  • Maximise recovery and promote reablement – Service users will receive care, support and risk management provision provided by the most appropriate practitioner to protect, promote and prevent.
  • People receive care at the right time in the right place closest to where they live.
  • Empower people to be in control of their lives and lead independent lives
  • Promote the values of self-management, choice and control
  • Promote Equal Opportunity and Accessibility
  • Care and support aim to focus on interventions using a psychological/ trauma informed approach that takes into account people’s past and current life experiences.

The following objectives will underpin the ethos of the Recovery and Reablement service

Protect- Rochdale Borough Council (RBC) recognises that vulnerable adults within Heywood, Middleton and Rochdale may be eligible to have their needs met in accordance with the Care Act however their lives may not be stable enough to maintain engagement. The objective is to engage with, protect and safeguard people who have experienced multiple disadvantages or are seeking redress from ACE, CSE or any other criminal proceedings.

Promote- by taking a more assertive approach to working in partnership with Pennine Care Foundation Trust Enhanced Access service, statutory and non-statutory providers to promote the well-being, independence and recovery of these groups.

Prevention- Work towards the prevention or delay of the need for care and support under Section 2 Care Act 2014 by actively working with people, carers, their networks and local communities with the aim of developing resilience and help promote self-reliance and independence.

Eligibility criteria

Criteria will consist of any of the following interrelated complexities, except where the primary problem is alcohol/drug dependency with no other.

  • Adults aged 18 and over
  • Young people transitioning to adulthood
  • Adult Criminal Exploitation (ACE)
  • Child Sexual Exploitation (CSE)
  • Housing Issues, specific to ongoing Homelessness/without appropriate accommodation
  • Ongoing Alcohol/Substance Misuse (if primary – to be referred to Turning Point)
  • Low level Mental Health issues
  • Experienced Trauma as a Child/Adult
  • Domestic Abuse  (Perpetrators and Victims)
  • Non engagement with services/agencies/partners
  • Unwilling or excluded from previous alternative services

Other criteria may also be related to the above and include:-

  • Has or is at risk of developing Mental Health issues (and is not being supported by any other mental health services) for example is suggesting or has thought about risk of harm to self or others behaviours, suicidal ideation. If there is current planning or intent the person is to be referred to A&E (Mental Health Liaison)
  • Significant relationship problems and this is manifesting in clear signs of showing of suffering from complex personality issues
  • They have a complex lifestyle including
    • Chaotic behaviour,
    • Is suggesting they are using substances and / or alcohol which is / has become problematic for them (but are not ready or do not know how to access services),
    • Issues with social networks (friends / family / socially isolated / lonely) which are affecting everyday living
    • Without appropriate housing and living arrangements / hoarding / severe environmental issues
    • Difficulties engaging with previous support and services
    • Debt management issues which are causing stress and worry, (for example at immediate risk of eviction)
    • At risk of ACE
Referral process

New referrals will initially be received by the Adult Care front door contact number and signposted to the Recovery and Reablement tray within ALLIS if criteria met.

Referrals from Adult Care Services and external partner agencies will be managed via the Recovery and Reablement service central inbox and/or the ALLIS system direct to the service by prior agreement.

CSE referrals will be accepted via the Asset Meeting, Complex Safeguarding Team and Sunrise Partnership Meeting and will be managed through the existing single point of entry. ACE referrals will follow the same pathway.
