About
The Family Health Link Worker service provides continuing holistic support to families of children with complex healthcare needs.
What we do
The service supports families to:
- Coordinate appointments effectively, ensuring regular attendance and easy access.
- Seek more effective transport options to and from appointments.
- Provide impartial advocacy to families, to ensure a full understanding of health input and the expectations of home strategies.
- Explore local services, and access them accordingly.
The service is designed to reduce the number of families in Rochdale, Middleton and Heywood who do not attend appointments for whatever reason, this helps to free up clinicians office time, which can be time consuming and costly for health services.
How can you access the service?
The service is not operated through self-referrals, families are only able to access after a referral is made by a health practitioner within the Children's Acute and Ongoing Needs Team.
The Befriending Project
This is a service which provides holistic support to families with children who have complex health needs, helping to build resilience. The project supports families with children who have specialist educational needs or disability (SEND).
