We provide mental health assessments and ongoing care for you if you have dementia or other mental health conditions such as depression, psychosis and severe anxiety.
We also work with people of any age who suffer from dementia.
If you have spent time in hospital because of your condition, we can support you when you are back at home to help you to avoid going back into hospital.
Our aim is to help you live as independently as possible.
Some of the ways we can help you include:
• Assessing and diagnosing your condition
• Providing medical care
• Psychological (talking) therapies
• Group and one-on-one therapy
• Occupational therapy
• Providing support and information for your carers
• Inpatient services (for 24 hour care)
We provide care in:
• Your home, including residential and nursing homes
• outpatient clinics
• community hospitals
• GP surgeries
We also work with external services such as social services and support your carers and family members with education, practical support and therapy.
We will treat you with dignity and respect and provide care based on your individual needs.
Once you have been referred to us, we will offer you an appointment.
During the first session we will start the assessment process and discuss an appropriate care plan to meet your specific needs.
This initial session is also an opportunity for you to talk about your thoughts, worries and expectations, and for us to be able to answer any questions you may have.
