Applicants must be 50+ with a disability or People are over the age of 60
How are referrals/applications made to your service?
Referral received (and Risk Assessment), Activities Team (AT) acknowledges referral and sends letter inviting them to Scheme for a Taster Day and Initial Assessment Activities Manager (AM) to carry out Initial Assessment and review Risk Assessment, with points system to determine: • Whether client is suitable for the service • Level of need (Low/Medium/High along side criteria in points system) • If client needs signposting to an appropriate agency and inform Social Services if referred • Position on waiting list if service is running at capacity.
