Information and advice on Adult Social Care and support

When a young person reaches the age of 18 the responsibility for providing social care support transfers from Children's Services to Adult Services. When a young person reaches adulthood the services provided may change. This could mean that they are no longer eligible for the same services, or that different services may be more suitable.

Under the Care Act 2014 local authorities must, from April 2015, provide an information and advice service on the adult care and support system. This should include information on types of care and support, local provision and how to access it and how to raise concerns.
