Moving from children to adult services for young people with an education, health and care plan

Young people and their families/carers approaching adulthood with a disability will be thinking about their future. They may think about what happens when they leave school or college. How they can be as independent as they want to be to live a fulfilled life. This can be an exciting time in their life but the changes also might be worrying.

To help make this transition as smooth as possible, new information has been launched on the Local Offer called Transition to Adulthood.

This new area of the Local Offer is packed with useful information such as top tips for parents, carers and young people when thinking about transitions - written by those with lived experience, as well as information Luton Councils different adult social care teams, when a transition to adulthood will take place and how we will support young people and their families carers during this important stage in their life. There is information about transitions from children to adult health services and from school to college.
