Learning Disability Team

The Community Learning Disability team works with people with learning disabilities to find the best support for their individual situations.

The Community Learning Disability team is made up of social workers, enablement workers and community nurses working with people over age of 18 who have a learning disability. The team is integrated, which means that we share our knowledge and expertise with each other to bring people a better service.

People with a learning disability and their carers no longer have their care needs looked at in isolation, but instead the integrated team will look at all aspects of a person's life.We refer to the way we work as ‘person-centred’ which means that the person with a learning disability is treated as an individual and their needs and wishes are right at the centre of their care. Person-centred care means we continually listen and learn and act on what is important to them now and in the future. Listening to the person’s carers, family and friends is important too.

What services can the team provide?

The Community Learning Disability team works closely with other organisations and services for people with learning disabilities.

Depending on the person’s individual circumstances, this may involve help about housing options - for example the supported living service - to help people access services and to meet their potential.

How can I get help?

To access the support of this team the person with a learning disability must qualify for support and then have an assessment. We then work with the person to identify the best support for them.
