Aims of the Integrated Community Diabetes Service

The service aims to provide an easy access specialist nurse-led service in a community setting. Our philosophy is to support patients and health care professionals to manage care safely and effectively in the community. By helping you manage your diabetes we aim to prevent you needing to be admitted to hospital and ensure that all patients have support to manage diabetes at home or in the community where possible.

Who delivers the services?

The service is delivered by a team of diabetes specialist nurses, (DSN's) diabetes support workers, dietitians, podiatrists (all are experienced in diabetes and delivering diabetes education) and administrators. Some of the team members speak Urdu and Bengali as well as English.
