Children With Disabilities Social Work Team

Lincolnshire County Council’s Children with Disabilities Team are part of the Special Education Need and Disability Service. The social work team provides a statutory service for children and young people with a severe and profound disability. Services may be available to those that are eligible after being subject of a social care assessment. Referral Details

This criterion identifies those children and young people that should be referred to specialist services and those that should not. The Customer Service Centre team manager screens all referrals to social care and makes the decision as to the most appropriate team to assess the needs of the child/young person as sometimes the needs of the child, young person and parent/carer can be met through other agencies or services offered by Lincolnshire County Council.

When the Children with Disabilities Team receive a referral they decide if it meets the eligibility criteria for accessing services provided by the Children with Disabilities Team.

Within 24 hours of a referral being made to the Customer Service Centre a Social Worker will be allocated to complete an assessment of need. The assessment can take up to 40 working days to complete but can be shorter in duration if it is identified that there are currently no unmet social care needs and other agencies would be more appropriate to offer support.

The social worker will need to find out information about the child, young person and their family in order to help decide what sort of help and support they require.

Once the assessment process is completed and services have come online the Social Worker will arrange for regular Child in Need reviews to be undertaken so that we can ensure that your child's needs continue to be met. Part of this ongoing process includes the Social Worker undertaking regular visits to see your child at home, school and where applicable at the short break setting.
