Home From Hospital Service

Our service is free and available to those who are aged 18+ and:

  • have low level need (we do not provide personal care).
  • live in County Durham or Darlington and are able to be discharged from hospital (including from accident and emergency departments) but may have anxieties about returning home.
  • if you have been referred from either County Durham Hospital Social Worker teams or Darlington RIACT Plus multi-disciplinary teams, to provide low level support to an adult at home who has recently been discharged from hospital.

Examples of the types of support include:

  • ensuring availability of food on return home.
  • ensuring the house is warm and tidy on return home.
  • meet and greet you on return home service (where transport is not provided).
  • pop in’s to make sure you are settled.
  • support you to develop confidence and independence post discharge.
  • preparation of light meals.
  • prescription collection.
  • light housework (for example run hoover round, washing up, making sure kitchen is clean and tidy).

We accept referrals with your consent from hospital staff, GPs, Hospital Social Worker teams/RIACT Plus/TAPs multi-disciplinary teams, other Social Care teams, County Durham Single Point of Access and family/carers/individuals.

The Home from Hospital Service (HFHS) is a joint service between Durham County Council and Darlington Borough Council and provides time limited support in the home (usually up to a maximum of six weeks).
