Mental Health and Wellbeing Outreach Services, Home Group – Care and Support
Our Mental Health and Wellbeing Outreach Services are a range of community social support schemes funded for those both under secondary and primary care by Durham County Council, and three services aimed at tackling health inequalities funded by Durham Dales, Easington and Sedgefield Clinical Commissioning Group for those under primary care.
All are for people with a mental health illness/issue and include those who have both learning disabilities and mental health issues. The services fully integrate to meet individual needs, enhance wellbeing and offer choice and opportunity through a recovery focused pathway.
Different services are provided across different areas but work together as one county-wide service.
Volunteering – Local authority funded for Social Inclusion
This service provides:
- Opportunities within open access groups located within community venues and local communities.
- Volunteering opportunities in various roles defined with the volunteer.
- Access from the general public and various client groups.
- Support to acquire or build on your skills.
- Opportunities towards gaining employment.
- Support from the volunteer co-ordinator and mentoring worker.
- The opportunity to use volunteering to aid your own wellbeing and recovery.
Befriending/Peer Support
This service provides:
- Volunteers who befriend you to increase your opportunities for social inclusion and aid mental wellbeing stability.
- Volunteer Befriender who is matched to you.
- Support from the volunteer co-ordinator and mentoring worker.
- Peer support.
Personalised services through a Personal Budget
This service provides:
- Support to those with enduring mental health issues on a one-to-one basis through a direct payment or virtual budget.
- The right amount of hours depending on the outcome of your assessment carried out your Care Co-ordinator.
- You'll have greater choice and control on how you would like to be supported in achieving your desired outcomes.
Physical Activity Service
This service provides:
- An increase in physical activity, leading to an improvement in mental health and wellbeing.
- An increase in more healthy eating.
- A link to smoking cessation classes.
- Increased interaction and stability through the use of community.
Open Access Groups
Provides support for between 5 - 15 clients depending on the group activity. The groups are all client led/constituted so the clients have the opportunity and choice to become as involved as they wish.
One-to-one Outreach Support
Support is provided on an individual basis for up to 25 clients. The support is mainly delivered in the Easington area although where we are able, we will provide support in other areas.
