• Team of Family Support Workers who offer interventions to families with lower  level needs, with the goal of identifying a Lead Professional or community anchor to provide ongoing and sustainable support to families
• The team also provide direct work with a family for up to 12 weeks
• Complete an Early Help Assessment if required
• Identify existing and appropriate professionals in the network
• Coordinate initial Team Around the Child / Family meetings
