Information about transitions for parents and carers

Transition is what we call the journey from being a child to being an adult.

For a parent/carer, this can be a confusing time, with lots of questions and not being sure what happens next. The wiki diagram below sets out what happens during a child’s transition, as well as where to get further help and support.

If you have any questions, there are lots of people you can ask. Whether they are your child's social worker from the Children and Young People Disability Service, health staff, independent organisations or peer support.

Some of the support available for parents includes:
